- Project Description: The Veterans Health Administration (VHA) deployed a Web-based portal that enables veterans and VHA staff to create and maintain personal health records for veterans. Veterans can easily track, record and graph metrics like weight, blood pressure and blood glucoseand keep their health records current. A customized summary of all relevant information can be created with a few mouse clicks and printed for sharing with the patient's healthcare provider. Veterans have also used the system to order more than 5,000,000 prescription refills. The program has more than 539,000 registered users.
