Project Description:
Meridian Health Plan's Care Coordination Module helps clinical and non-clinical staff collaborate on the coordination of a member's care, identify high-risk members and forecast their healthcare needs and accompanying costs. Designed as part of Meridian's Managed Care System (MCS), the module relies on predictive modeling software to automatically detect these high-risk, high-cost members so staff can better manage, coordinate and prioritize care. By proactively intervening with members before their health worsens significantly, Meridian has decreased emergency room visits by 15 percent and cut the amount paid in medical claims by 14 percent. In addition, by using MCS to analyze the medications of members, pharmacy technicians have reduced the average number of unique drugs dispensed from 8.1 to 7.6-a cost savings for Meridian of 5 percent.
