Project Description:
Kindred Healthcare's "Connecting the Care" business and technology transformation revolved around three principles. First, it established a foundation of digitized and interconnected data, promoting integration of data among hospitals, pharmacies, labs and post-acute partners. Second, it applied scientific and predictive analytics to that data. Bolstered by a partnership with SAP and its HANA platform, Kindred quickly aggregates, normalizes and analyzes data on more than five million post-acute patients. Third, they instituted a heavy bias toward mobile computing. This mobile-first approach extends from the field (a sales force using a mobile version of Salesforce.com) to operations (a home health workforce that coordinates the activities of more than 45,000 mobile home health providers) to their therapists (a mobile rehabilitation and physical therapy workforce serving 500,000 patients a year). "Connecting the Care" has helped Kindred reduce hospital re-admissions, reduce patient length of stay and deliver higher quality care at lower price points.
