Mount Sinai Health System

Project Description:
The Mount Sinai Health System began work in early 2015 on the new Health Information Exchange (HIE) system which was designed and implemented by Mount Sinai Health System's Information Technology department to provide several main functions. The team set out to provide the new HIE system which would support their seven hospitals, community partners, and regulatory organizations. By establishing a process of sending electronic messages, which are then matched to a database/registry of records based on specific criteria, the new HIE system merges the records with the existing data in the system. Each partner organization who is connected sends the HIE a message in a Continuity of Care Document (CCD) format, at which point the HIE consolidates and stores the CCD's in an electronic filing cabinet. The new HIE Portal provides the ability to securely review the patient records by internal clinicians, community providers, and affiliates in a timely fashion which eliminates the need of faxing patient records and reduce data duplication.
