Building on its existing EHR (electronic health records) system, Holyoke Medical Center (HMC) integrated leading-edge technology from QPID Health that locates and synthesizes critical patient facts from electronic health records for timely identification of behavioral health patients at risk for revolving emergency visits and readmissions. HMC developed a comprehensive behavioral assessment form with mandatory key fields to track referrals and services for its behavioral health patients. QPID provided the analytical capability to query these fields. HMC introduced robust care navigation so patients identified using QPID software receive targeted interventions, are referred to the right services and successfully follow through on their care plans. Within 12 months, Holyoke Medical Center reduced 30-day emergency department revisits for patients with primary or secondary behavioral health diagnoses by 25 percent.
