2019 Winner Profile

Kaiser Permanente

Project Description:
The Advance Alert Monitor (AAM) is an early warning system that predicts a patient's likelihood of deterioration outside the intensive care unit in the next 12 hours. The system continually analyzes the patient's electronic health record to generate a score, based on more than 70 factors including three predictive models. When the score reaches a threshold, it triggers an alert to remote RNs. They review the patient's medical record and escalate cases that would benefit from intervention. AAM has decreased hospital mortality by 20%, reduced unplanned ICU transfers, and improved end-of-life planning. The program generates approximately 30 alerts each day, and about 1 in 10 patients transfers to the ICU.
